What Is High Intensity Focused Ultrasound (HIFU)?

High Intensity Focused Ultrasound (HIFU) has joined the mainstream options for the treatment of prostate cancer.  HIFU is a noninvasive treatment with the capability of curing prostate cancer, without significant side effects.  HIFU is an outpatient procedure that has men returning to social and business functions in a matter of days.

HIFU has been used for well over a decade in more than 49 countries. This unique technology recently received FDA approval, and is now available in the U.S.  This is a milestone for prostate cancer treatment. This new technology is accompanied by a breakthrough in imaging technology, 3 Tesla MRI which allows us to direct therapy to only the areas of the prostate with disease. This is known as focal therapy. This is similar to lumpectomy used to treat breast cancer. Only the breast tissue with disease is treated.

Another way of thinking about focal treatment is cutting the brown spot out of an apple. By eliminating the bad spot only, normal prostate function is maintained. On the other hand, radical prostatectomy removes the entire prostate (throws away the apple) and the urinary tract must be reconstructed. This can lead to urinary control and sexual dysfunction issues.

HIFU treatment does not disrupt sexual functioning or urinary control. HIFU works by directing focused sound waves to create the precise amount of heat to destroy cancer cells. This can be compared to using a magnifying glass to focus the sun’s rays on a piece of paper, resulting in a fire. You would not however, feel the heat when passing your hand under the magnifying glass.

Sonablate (R) 500
The Sonablate® 500 is a medical device that uses HIFU to thermally ablate the prostate using a transrectal probe.

Status of HIFU in the US


As of October 13, 2015, SonaCare Medical Receives FDA Clearance for its Sonablate® High Intensity Focused
Ultrasound Prostate Ablation Device. The Device will start distribution in October.

The authorization of use of the Sonablate® in the U.S. is a milestone for non-invasive prostate care and a tremendous gain for men's health.

Formerly, the device was only available for use in Canada, Bermuda, the Bahamas, The Dominican Republic, Argentina and Europe.

Brief History of HIFU

  • HIFU research began in the 1950s in Indianapolis, IN.
  • In 2004, the first International HIFU Centers opened in the Americas, outside of the United States.

These are T1.5 Tesla MRI's. The first MRI shows a patient prior to the HIFU procedure.

The second image shows the prostate 2 weeks Post-HIFU. The entire "meat" of the prostate is gone.

The patient is able to urinate with good control and has retained sexual function. He is a relatively young person.

Who Qualifies for HIFU?

Ideal HIFU candidate:
  • Localized prostate cancer
  • PSA < 10
  • Gleason < 7
  • Prostate Volume 40 cc
  • Other patients may also qualify and should discuss their specific case with a physician.

The Sonablate® HIFU Procedure

  • 1-4 hour outpatient procedure, depending on size of prostate
  • Spinal/epidural anesthesia with light sedative
  • Therapy is delivered through a transrectal probe after patient is numbed from the waist down
  • 1-2 hour recovery at HIFU treatment facility
  • After procedure, most patients resume normal lifestyle within a few days
  • In most cases, HIFU is a one time procedure. It is non-surgical, radiation free and has minimal blood loss.

  • PSA
  • Gleason
  • Prostate Volume (very important)
  • Other therapies/treatments and any relevant medical history
